Looking for a family doctor?

If you or someone you know in Hamilton does not have a family doctor the GHHN can help!

The Greater Hamilton Health Network and Hamilton Family Health Team are working to connect eligible Hamilton residents with a primary care provider.

Individuals can contact the Hamilton Family Health Team directly, or community service providers can submit a referral.
